If the environment variable POSIXLY_CORRECT is set dpkg-source -x fails.
This is because the diff files dpkg builds don't contain enough information
for a POSIX patch to know where to create new files.
Ideally we would generate diff's that a POSIX patch would be able to apply.
The quick fix for dpkg-source is to invoke patch with the environment cleared.
That would probably be the right thing in any case since patch checks some
other environment variables and dpkg-source really wants consistent
deterministic behaviour, not what the current user happens to prefer.

This bug appears to be fixed at some point before 1.6.6:
delete $ENV{'POSIXLY_CORRECT'};
It would be nice to generate posixly correct patches, but I'm not sure that's
possible. As long as dpkg-dev depends on GNU diff and patch though it's
probably enough to just do this.
