That's because glibc 2.1 is db2 and db1 headers are there for
compatibility only.

ding ding ding...we have a winner...I knew there was a good
reason for it...or rather I hoped there was... :)

Incidentally, /usr/include/db.h should be able to substitute for ndbm.h, since it declares all the functions that ndbm.h does.
