If you work as audio or video mixer, make a note of the time if you lose sound,
video or DV capture.  Append such notes to a README for the capture, so that
post-processors save time, and have a better chance of finding and fixing/masking
the problem.

The README should be located in the same directory as the capture, and have
an obvious name, e.g.:

title_15.45_13092005.dv	 <-- capture
title_15.45_13092005.README  <-- README
