Bug#408462: /etc/default/apache2 NO_START option

There are 2 problems with /etc/default/apache2.  The first is that it
exists.  Are not daemons supposed to be enabled via runlevels?
I would consider providing a redundant mechanism for enabling/disabling
a daemon to be a bug because it impairs functionality that is already
supposed to exist.

The second bug is that it says in the comment "0 = start on boot; 1 =
don't start on boot" however if this is set to "1" the daemon will not
start ever, not even manually, and will not even give output as to why
it is not starting.
