I was testing from an external machine.

I know, I know, I should have posted "[ I solved it! read below ]" at
the top of the mail to which you are replying.

The network setup was rather complicated, the printer already had a
default gateway of a different NAT box, so its return packets were going
out with a different internet IP address.  So my port forwarding had
probably worked all along!  Yeesh :o.
