I know who the 4th one is :)
The first two though are generated by a _buggy_ resolver in webalizer.
I usually simply let apache resolve the hostnames, untill webalizer gets
fixed correctly ofcourse ;)

BTW: http://6bone.informatik.uni-leipzig.de/ipv6/stats/stats.php3 for
the
"Official 6bone Webserver List". Actually should be s/6bone/ipv6/ but
what the heck..
