I managed to reproduce this by pressing Up Arrow after Alt-Tabbing into
qpdfview.

Yep, just a coincidence. I've had this weird document focus thing happen
several times since I made that change.
Unfortunately, the weird behaviour is not 100% consistent. It happens a lot,
but not all the time. It's not random, but I haven't been able to figure out
exactly what triggers it.
