installing and configuring dpdk, 5.10 and 4.19 provide ib_uverbs and
mlx4_ib but 5.4 does not.
* What exactly did you do (or not do) that was effective (or

updating to 5.10 fixes the issue

You mention it above, that it works with 5.10 based kernel.
5.4.8-1~bpo10+1 is not anymore something supported in Debian, current
will be 5.10.103-1~bpo10+1 for backport kernel in buster.
In fact for the clound kernels the INFINIBAND configs for
HyperV/Azure were enabled in 5.6.14-1 (cf. #958300).
